Emergency rooms in the USA are under increasing pressure to provide emergency care to all, even if they are non life threatening. This trend in approaching ER for every singly problem is costing the healthcare system a large amount of costs, with the result that the number of ER services and hospitals are decreasing with increased pressure on the remaining healthcare facilities.Other contributing factors are not helping either. In addition to this the issue of increased alcohol consumption and ER admissions is also beginning to negatively affect the quality of healthcare. Nurses are under more pressure than ever to handle ER efficiently, which is showing no signs of decreasing the number of unnecessary ER admissions.Alcohol and Drug Abuse Are Main Abusers of Emergency Room ServicesMany reports now confirm that Emergency room services are among the most misused healthcare services employed by the patients. Of the many problems with which the patients come into the ER with, the most co mmon are due to alcohol and drug abuse.According to McGeary (2000) â€Å"chronic illicit drug use is a positive as well as a significant determinant of the probability of utilizing the ER.† (McGeary et al, 2000) Substance abusers were 30 percent more likely to utilize the ER services than any other emergencies. (McGeary et al, 2000)The above statements and study results point out to many issues and findings. Firstly, that drug users or abusers are the most prominent patient crowd of the ERs. This leads to the next conclusion that in order to improve ER services, we must be able to identify and care for the main target patients, who are drug users, alcoholics and such related substance abusers.ER is also a most likely place where screening for psychiatric symptoms and drug abuse can be identified, and further treatments carried out. In this way, an ER can be looked at as a diagnostic, screening and treatment facility for those who are substance or drug abusers, with or without psychological symptoms.This also means that in order to improve services in the ER, the prime patient population in focus should be such people, and effective measures and policies must be introduced to provide care for this group. The costs utilized in the education and prevention of alcohol abuse is far less than the costs that it takes to treat alcohol cases.The trend in opting for emergency visit initially may have been limited to emergency cases, but in the recent years has grown to include insignificant calls that may not need emergency initiatives. The problem lies in lack of awareness among people about what constitutes an emergency and what not.To many this increase in the recent years in the emergency room entries and calls are due to the increasing size of the populations, the reduction of services and number of hospitals, the decrease in the number of nurses, and the problems of not having insurance and coverage in the American population. The current concept of ER has changed to become a place that provides round the clock services for all healthcare related problems instead of a service that is essentially made to save lives in critical danger.The complications are leading to more admissions, with the danger that many patients who may be in actual need of the critical treatment may experience delays in their treatment, leading to significant morbidity and mortality. (Severson, 2000) The current statistics reveal that one in five of the visits to the ER are for non-urgent conditions. (Osheraoff, 2004)Of the most common abusers of the ER system, men, poor families and African Americans are seen to abuse them the most. These numbers are high in all of the American states. In California, this includes a quarter million of the Californian adults and 60,000 children.The increase these admissions and care treatments cost are six times more than the normal clinical expenditures. Of the abusers, the non citizens were more likely to utilize the ER service s when compared to the citizens. All these increases in the numbers are thought to take place due to the reasons mentioned above. The ER services nation wide are in deep trouble. This was coupled by the closing of 425 emergency departments, 700 hospitals and 200,000 beds. This means that as the number of ER services is decreasing, the remaining services are experiencing a sharp rise in the admissions and therefore more work with lesser amount of quality provision. (Brown, 2006)What many of the Americans refuse to acknowledge is that the ER visit is also costly for them, apart from the increased cost that the healthcare sector pays. This trend to have every thing taken care of in the ER is usually due to two reasons on the patients’ behalf. One is to save oneself from waiting fo r an appointment with the primary healthcare provider, and second is the lack of awareness among the Americans as to what constitutes an emergency. In 2004, there were near 2 million admissions in the ER for drug related mishaps, of which 1.3 million were due to drug misuse or abuse. (SAMSHA, 2006)Of the 1.3 million, 30 percent comprised of illicit drug abuse cases, 25 percent were related to over the counter and prescribed medications, 8 percent were alcohol abuse cases in people under age 21, and the rest included combinations of illicit drugs, pharmaceuticals and alcohols in various combinations and percentages.The drugs that were primarily involved comprised of cocaine, marijuana, heroine, stimulants, amphetamines and methamphetamines, PCP, Ecstasy and GHB in the largest amounts. However, many argue especially from the Asian American community, that he’s not just a fad or trend but a full blown star athlete, pushing the envelope not just for Asian Americans, but athletes across all social classes, races, and creeds. So really, with all the media hype and hysteria, who is Jeremy Lin? Why is he the chosen one? More importantly, what has he been labeled and how has he faced this discrimination? What is the cause of this â€Å"Linsanity†? Before the question, â€Å"Why Jeremy Lin? † can be answered, we have to start at his roots. Jeremy Lin was born August 23, 1988, to a Taiwanese emigrant couple. He grew up playing basketball in Palo Alto, Northern California. He excelled not only as an athlete in his young school years, but was also an exceptional student. He went onto Harvard, a fallback school, and continued his academics, majoring in economics. While at Harvard, Lin played for the Ivy Basketball League and quickly made a name for himself. This is where he first encounters racism and discrimination. In short interviews throughout his college days, Lin spoke of his opportunities and how his race had been a factor in his recruitment. â€Å"I’m not saying top-5 state automatically gets you offers, but I do think (my ethnicity) did affect the way coaches recruited me. I think if I were a different race, I would’ve been treated differently. † He was never drafted after graduating college and continued to play basically for free as a D-League athlete with the Golden State Warriors. A few days later after a loss to the Orlando Hornets, ESPN’s Max Bretos reported during a taping of SportsCenter, â€Å"We have found a Chink in the Armor†. Floyd Mayweather Jr., a professional boxer and fellow athlete tweets, â€Å"Jeremy Lin is a good player but all the hype is because he’s Asian. Black players do what he does every night and don’t get the same praise. † â€Å"Linsanity† was bringing in a negative backlash that Lin was all too familiar with. He looked past it, and went even as far as to forgive. â€Å"I expect it, I’m used to it, it is what it is,† says Lin. On the â€Å"Chink in the Armor† slur, Lin responds, â€Å"I don’t think it was on purpose or whatever. At the same time, they’ve apologized, and so from my end I don’t care anymore. A case in point, the corporate world of advertisement tried to cash in on â€Å"Linsanity†. Ben and Jerry’s Ice Cream Factory tried capitalizing on Jeremy Lin by coming up with a new flavor called â€Å"Taste the Linsanity†. The ingredients were basically vanilla ice-cream with a hint of caramel and the addition of fortune cookies. Many members of the Chinese/Taiwanese American community cried foul in this instance, as it did play to some degree more or less on Lin’s Chinese/Taiwanese heritage. In response, an Asian American protestor went as far as to picket the Ben & Jerry’s store on Haight and Ashbury in San Francisco. He held a sign that read, â€Å"Taste of Lebron – Watermelon and Fried Chicken in every scoop†. Now piggy-backing on another racial stereotype isn’t exactly politically correct, but it did bring awareness. â€Å"Taste of Linsanity† quickly made its way off of shelves and into trash cans. However, it wasn’t only corporate America playing on stereotypes for publicity. It was the Asian American community itself looking to address positive stereotypes. In the Youtube video â€Å"Superior Lintellect† by studio64comedy, creators Lawrence Kau and Kunal Dudheker (both Americans born of Asian descent) portray Lin solving complex math equations in his mind in order to best his competition during tough situations on the basketball court. Each playback of Lin’s on-court moves are narrated by a forced Chinese accent, and are accompanied with on-screen quadratic formulas and physics theory. It’s no doubt that Asian Americans are poking fun at the â€Å"Asians are good at math† stereotype. But yet in some ways, it just feeds fuel to the fire that stereotypes are okay. The question ultimately comes down to, â€Å"How far is too far? † and â€Å"Who can and who can’t stereotype against Asians? † In conclusion, Jeremy Lin is a unique individual. Yes, he is a Harvard graduate. Yes, he is an incredible athlete. Yes, he is a fiercely loyal Christian. And finally; Yes, He’s Asian. But despite all his unique attributes, why is his race the only thing that seems to overshadow his qualities? After all this time, when the name Lin appears on TV based on performance and skill, the media is still focused on what school he went to and what GPA he had or whether he was Chinese or Taiwanese. Is it the scarcity of Asian Americans in the media that makes it so socially acceptable to shift focus? Is it the Asian American fans all across the country coming to support him, regardless of team pride that focuses the media’s attention to his ethnicity? Is it the blatant ignorance of the general populace? Maybe it’s all of that and then some. But regardless of where all of this attention originates, it is safe to say that Jeremy Lin is on to something very special. He’s allowed Asian Americans into mainstream sports and has brought a positive light to an arena once absent of it. After I graduation from high school, I had plan to do several things. The most important thing I plan to do is go to college, because there are so many other detractions other than a degree, from a college education I plan to pursue higher learning, to begin a career, and to make history in my family. Making history in my family is the most important thing I hope to gain from a college education. My mother was the one parent that parents graduated from college, I have two brothers and a sister who did attend college which motivated me to want to go to college; my other relatives a least graduated from high school. Taking a look back has motivated me to reach farther. Being able too graduate of a catholic department system and also a highly respected person in my senior class. I plan to make history in my family by being the first also to get a Degree in History. Standardization of Polyherbal Formulations for Drug Analysis Standardization of polyherbal formulations is essential in order to assess the quality of drugs, based on the concentration of their active principles (markers or standards). As Mentat tablet (MT), a herbal formulation consist of natural multi-ingredient formula with active constituents responsible for treatment of neurological disorder and improvement of mental health, therefore it is worthwhile to take it as a model for standardization of polyherbal formulation. In this work, an ultra-performance liquid chromatography electrospray ionization tandem mass spectrometry method has been developed and validated for simultaneous quantification of 28 major bioactive constituents. Multiple-reaction monitoring scanning was employed for quantification in positive and negative mode. The analysis was accomplished on Waters AQUITY UPLC BEH C18 column with linear gradient elution of acetonitrile 0.1% formic acid with water 0.1% formic acid solutions at a flow rate of 0.3 mL/min. The comparative a nalysis of samples was carried out by hierarchical cluster analysis and principle component analysis. The proposed method was applied to analyze 4 different batches of marketed formulation with acceptable linearity (r2, 0.9984-0.9999), precision, stability and recovery (RSD ≠¤ 3.74 %), evaluated under optimum conditions. The developed method is capable of controlling quality of polyherbal formulations having similar markers and herbs. Herbal medicines (HMs) refer to one herb or complex mixtures, which usually contains hundreds of chemically different components. Their curative effects are principally based on the synergic effect of their multi-targeting and multi-ingredient preparations [1, 2]. Consequently, quality control becomes a troublesome issue for crude drugs and their medical preparations. Therefore, the method that employs pharmacologically active components to evaluate the quality and authenticity of the complex preparations is confronted with severe challenges and better analytical strategies to assure their efficacy, safety, and consistency are in great demand [3]. Moreover, the chemical compounds in the poly herbs in HMs products may vary depending on harvest seasons, plant origins, drying processes and other factors. Thus, it seems to be necessary to determine most of the phytochemical constituents of herbal products in order to ensure the reliability and repeatability of pharmacological and clinical research, to perceive their bioactivities and eventual side effects of active compounds and to enhance product quality control [4, 5]. Currently, selection of a single or a few specific components from a certain herbal medicine as markers for quality assessment is a widely applied strategy. However, it cannot afford sufficient quantitative information for the other medicinal compositions and cannot accurately reflect the quality of HMs products. All the HMs compositions play important roles in the therapeutic effects. Therefore, selecting multiple constituents from different medicinal herbs as evaluation markers has been gradually applied for the quality control of HMs [6, 7]. Mentat tablets (MT, commercial product) is a polyherbal medication with each tablet composed of extracts (136 mg of Bacopa monnieri, 70 mg of Centella asiatica, 52 mg of Withania somnifera, 52 mg of Evolvulus alsinoides, 52 mg of Nardostachys jatamansi, 50 mg of Valeriana wallichii, 50 mg of Embelica ribes, 50 mg of Prunus amygdalus, 42 mg of Acorus calamus, 36 mg of Tinospora cordifolia, 36 mg of Terminalia chebula, 36 mg of Emblica officinalis, 32 mg of Oroxylum indicum, 32 mg of Celastrus paniculatus) and powders (80 mg of Bacopa monnieri, 18 mg of Withania somnifera, 18 mg of Mucuna pruriens, 18 mg of Elettaria cardamomum, 18 mg of Terminalia arjuna, 18 mg of Anethum sowa, 18 mg of Ipomoea digitata, 14 mg of Zingiber officinale, 14 mg of Terminalia belerica, 14 mg of Myristica fragrans, 10 mg of Syzygium aromaticum). Natural alteration in preparation processes and climate affects the safety and batch-to-batch uniformity of HMs products. Highly sensitive analytical methods are thus required to identify ingredients and evaluate lot-to-lot consistence. To the best of our knowledge, there is no method have been reported for the simultaneous estimation of selected 28 multi-markers in herbals by UPLC-ESI-MS/MSand no such formulation approach has been explored on this important drug combination for quality consistency evaluation of this herbal preparation. Compared to conventional TLC, HPTLC, HPLC–UV method which will take a longer analysis time, UPLC-ESI-MS/MS method in multiple reaction monitoring (MRM), tandem MS scan mode offers detection effectively with more powerful approach, to rapidly quantify multi-ingredients in complex sample matrices due to its rapid separation power, low detection limit, high sensitivity, selectivity and specificity. The methods used or reported in literature only contained one or two compounds, without consideration of other active ingredients. This paper describes for the first time a simple, accurate and reliable UPLC-ESI-MS/MS method for the simultaneous determination of 28 bioactive multiple components from different polyherbs including: bacoside A (mixture of bacoside A3, bacopaside II, bacopaside X and bacopasaponin C), withanolide-A, withaferin-A, asiaticoside, madecassoside, jatrorrhizine, palmatine, magnoflorine, curcumin, gallic acid, protocatechuic acid, ferulic acid, caffeic acid, ellagic acid, rosamarinic acid, ursolic acid, catechin, apigenin, luteolin, quercetin, rutin, kaempferol-3-O-rutinoside, corilagin, chrysin and chlorogenic acid with single runtime of 10 min. The results indicated that the developed method is fast, sensitive and convenient to show the real quality of the polyherbal preparations. Therefore, this proposed method could be reliable and feasible for quality assessment of MT and all other herbal formulations manufactured by various pharmaceutical manufacturers with similar herbs combinations containing either these 28 markers or less. The method was also applied to analyze the multiple components in MT different batches from same manufacturer. Our results will facilitate the comprehensive quality control of MT and other similar related preparations. The quantitative results were further analyzed by multivariate statistical analysis i.e., hierarchical cluster analysis (HCA) and principle component analysis (PCA) to provide more information about the chemical differences and batch-to-batch uniformity.
